MEDICAL SERVICES

IHPC Medical Services can provide care management and coordination to help your members manage medical conditions more effectively.  Access to Nurse Care Managers promotes improved health status and outcomes and reduced need for more costly medical services. The goal is to engage members and assist them in setting and reaching self-management goals.

Chronic Condition Management:

IHPC Nurse Care Managers offer guidance and assistance to members in managing chronic conditions such as:

  • Asthma
  • Diabetes
  • Depression
  • High Blood Pressure
  • Coronary Artery Disease
  • Chronic Obstructive Pulmonary Disease

Members are encouraged to receive treatment consistent with evidence-based standards of care. Experienced RNs employ health coaching techniques to reach out and offer clinical guidance and support to foster healthy lifestyle improvements and better outcomes.

Medical Review & Pre-Authorization:
Medical treatment requests are reviewed to help insure members are receiving care that is appropriate, cost effective, and medically necessary. Physician reviews will be obtained for treatment requiring authorization that does not meet specified guidelines.

Population Management & Wellness:
Nurse case managers use tools that analyze medical claims to determine if members have received recommended preventive exams, screening and services such as mammograms, physical exams, Pap smears, vaccines and colonoscopies. If members are overdue, they will be contacted to encourage obtaining these medical services which promote early intervention. Members and providers have access to health education and resources through our web site.

Home Based Care Coordination:
CareHub programs can help members and their loved ones get connected to a variety of programs that provide support, guidance and assistance with navigating resources to improve their health. CareHub will assign a Community Health Worker to connect adults and children to needed medical services and social service supports such as:

  • Medical, Dental, or Specialty Care
  • Food
  • Clothing
  • Housing resources
  • Utility assistance
  • Child care
  • Financial assistance
  • Education and employment
  • Legal support
  • Domestic violence resources
  • Mental health care and Substance Abuse Treatment
  • Transportation support
  • Interpretation services
  • Asthma Case management referrals, education and home visits
